This Privacy Policy describes the types of information we may collect from you or that you may provide when you visit, download, or use our websites, or our mobile applications, including the Ivy Fertility Patient Journey App (collectively, the "Services"), and our practices for collecting, using, maintaining, protecting, and disclosing that information.

- When you interact with our advertising and applications on third-party websites and services, if those applications or advertising include links to this policy.
- Offline, where such information is subject to the California Consumer Privacy Act of 2018, as amended by the California Privacy Rights Act of 2020 (the "CCPA").

- Any third party, including through any application or content (including advertising) that may link to or be accessible from or on the Website.

Children
The Services are not intended for children, as defined under applicable law. Children may not provide any information to or on the Services. - Information that you provide by filling in forms on the Services. This includes information provided when you request or schedule a consultation, make an appointment, or contact us. This information may include your (or the patient's) name, email address, telephone number, date of birth, age, address, insurance provider, country, city, state, family type, partner information (including name, email address, date of birth, and telephone number), patient primary care physician/OB/GYN information (including provider name, telephone number, fax, and office name and address), and information related to the services you are interested in, how you heard about us, how long you have been trying to get pregnant, which doctors you have consulted about fertility issues, whether you are interested in help financing fertility treatments, whether you have health insurance (and, if yes, whether infertility treatment is covered), and any other information you would like us to know.
- Information that you provide when you fill out patient forms. This information may include information you provide when you fill out patient registration forms, infertility evaluation forms, patient contact and authorization forms, consent to release medical records forms, semen analysis instructions, media releases, genetic screening forms, authorizations for release of confidential information, and treatment consent forms.
- Information that you provide when you submit an online payment. This information may include patient information (including patient name, date of birth, account number, billing address, telephone number, and email address), cardholder information (including cardholder name, billing address, telephone number, and email address), reason for payment, and payment amount.
- Information that you provide when you register for an account with us or utilize our patient portals or mobile applications. This information may include your name, birthdate, marital status, religious affiliation, race, nationality, address, user ID, patient number, email address, password, telephone number, sex assigned at birth, generation, gender identity, driver's license, vaccination card, reference information (including your referring OB/GYN, referring MD, and doctor on record), demographic information (including your ethnicity, language, employment information, age, gender, sexual orientation, and social security number), lab results, appointments, patient instructions, historical vitals, treatment cycles, payment balances, receipts, financial quotes, portal activity log, information related to the center in which you are most interested, the name of the physician you would like to work with, the type of services you are interested in, and how far along you are in researching your IVF journey, medical records, flow sheets (including cycle description, cycle start date, LMP date, peak E2, and treatment outcome), history forms (including information related to male information, pregnancy information, medical history, environmental factors, and male infertility tests), prescriptions, messages, pharmacy name and telephone number, the contents of your registration forms (including information contained in your patient registration forms, social and family history forms, and genetics forms, including information related to your current health and risks), insurance information (including insurance card, insurance carrier, coverage, primary insurance, insured individual identity, insurance name, group number, policy number, effective date, and address), information related to individuals authorized to discuss your medical condition with a healthcare professional (including name, relationship, telephone number, and notes), information about your spouse or a partner (including name, date of birth, duration of relationship, and partner photo), and any questions or messages you would like to share with us or documents you choose to upload.
- Information that you provide when you invite a partner to utilize our Services. This information may include your partner's name and email address.
- Information that you provide when you apply to become a gestational carrier. This information may include your name, telephone number, date of birth, email address, age, whether you are currently married or in a committed relationship (and, if yes, how long have you been together, will he/she be a party to the surrogacy agreement, and, if married, how many years have you been married), whether anyone in your household, including yourself, has ever been arrested or convicted of a crime (and, if yes, an explanation), pregnancy history (including total number of pregnancies, total number of successful deliveries, the ages of your children, how many children you have delivered vaginally, how many children you have delivered via C-section, the length of time you carried each child, blood type, how many miscarriages you have had, if you have suffered any miscarriages, the number of weeks pregnant when you miscarried, complications you had with any pregnancies, whether you have experienced postpartum anxiety or depression after any of your deliveries (and, if yes, an explanation), whether any of your children have special needs (and, if yes, an explanation), medical history (including your height, weight, current prescribed medications, health conditions you currently have or have ever suffered from, other health conditions, whether you suffer from any medical/physical conditions (including sexually transmitted diseases) (and, if yes, an explanation), whether you use any tobacco products (and, if yes, which products), whether you are exposed to second hand smoke, whether you use any marijuana products (and, if yes, how frequently), whether you use any illegal drugs and whether you drink alcohol (and, if yes, how frequently), and surgical history (including whether you have had any abdominal or gynecological surgeries or medical procedures (and, if yes, the date and procedure), and any other surgeries performed and when).
- Information that you provide when you apply to be an egg or sperm donor. This information may include your username, password, name, address, telephone numbers (including cell, home, and work), date of birth, email address, gender, personal description, characteristics, and traits (including height, weight, hair color, eye color, blood type, race, ethnic background, skin tone, and hair type), language, educational information, current and previous occupations, social history (including religion, information related to tobacco use, alcohol use, and drug use, tattoos and dates acquired, information related to whether you are currently single, married, or living with a partner, whether you have discussed your interest in egg donation with your partner, and, if not, whether you plan to tell them, information related to criminal convictions and incarcerations, and information related to personal abilities, talents, and interests), partner information (including name, relationship, occupation, date of birth, telephone number, email address, and address), emergency contact information (including name, relationship, telephone number, and address), sexual history, employment history, personal medical history, family medical history, personal photo, and information related to why you are interested in egg donation, your preferences, goals, hopes, and dreams for the future, and how you heard about our program (and, if you heard about the program from a previous donor, who).
- Information that you provide when you apply for employment with us or join our talent community. This information may include your name, email address, telephone number, address, and resume.
- Your search queries on the Services.
Information We Collect Through Automatic Data Collection Technologies.
As you navigate through and interact with the Services, we may use automatic data collection technologies to collect certain information about your equipment, browsing actions, and patterns, including:
- Details of your visits to the Services including traffic data, location data, logs, and other communication data and the resources that you access and use on the Services.
- Information about your computer, mobile device, and internet connection, including your IP address, operating system, unique device identifier, and browser type.
The information we collect automatically may include personal information or we may maintain it or associate it with personal information we collect in other ways or receive from third parties. It helps us to improve the Services and to deliver a better and more personalized service, including by enabling us to:

We strive to provide you with choices regarding the personal information you provide to us. With the exception of the Global Privacy Control required under the CCPA, we do not respond to Do-Not-Track signals at this time; however, we have enabled mechanisms to provide you with the following control over your information:

State Rights
Nevada residents who wish to exercise their sale opt-out rights under Nevada Revised Statutes Chapter 603A may submit a request to this designated address: appinquiries@ivyfertility.com. However, please know we do not currently sell data triggering that statute's opt-out requirements.

Accessing and Correcting Your Personal Information
You may send us an email at appinquiries@ivyfertility.comto request access to, correct, or delete any personal information that you have provided to us. We cannot delete your personal information except by also deleting your account.
We may not accommodate a request to change information if we believe the change would violate any law or legal requirement or cause the information to be incorrect.
Data Security and Retention
We have implemented measures designed to secure your personal information from accidental loss and from unauthorized access, use, alteration, and disclosure.
Unfortunately, the transmission of information via the internet is not completely secure. Although we do our best to protect your personal information, we cannot guarantee the security of your personal information transmitted to or through the Services. Any transmission of personal information is at your own risk. We are not responsible for circumvention of any privacy settings or security measures contained on the Services.
We retain your information for as long as needed: (i) to conduct business with you; (ii) as needed for the purposes outlined in this Privacy Policy; and (iii) as necessary to comply with our legal obligations, resolve disputes, and enforce any agreements.

You may authorize an agent to submit a request on your behalf if you provide the authorized agent with written permission signed by you. The Company may require you to verify your identity directly with us and confirm that you provided the authorized agent with permission to submit a request on your behalf.
